> The IPI calculator scores the International Prognostic Index for diffuse large B-cell lymphoma – age, stage, LDH, performance status and extranodal sites – and gives the standard and revised (R-IPI) risk groups for patients treated with R-CHOP.

## Inputs

- Age over 60
- Ann Arbor stage III or IV
- Serum LDH above the upper limit of normal
- ECOG performance status 2 or more
- More than one extranodal site

## Results

- IPI score
- IPI risk group
- Revised IPI (R-CHOP era)

## Formula

```
Score = sum of the points for each item
```

## Explanation

The IPI was published in 1993 and remains the main prognostic tool in aggressive lymphoma. The revised IPI uses the same five factors but regroups them for the rituximab era.

### How to use the IPI Calculator

- **Age over 60**
- **Ann Arbor stage III or IV**
- **Serum LDH above the upper limit of normal**
- **ECOG performance status 2 or more**
- **More than one extranodal site**

Results update as you type, and every field accepts the units shown in its menu.

### How it works

Each answer carries the points shown in the published score. The calculator adds them up and maps the total to the published interpretation.

## Worked example

Inputs — Age over 60: Yes; Ann Arbor stage III or IV: No; Serum LDH above the upper limit of normal: Yes; ECOG performance status 2 or more: No; More than one extranodal site: No.

Results — **IPI score = 2 / 5**; IPI risk group = Low-intermediate; Revised IPI (R-CHOP era) = Good (about 79%).

## Key terms

- **DLBCL**: Diffuse large B-cell lymphoma.
- **ECOG**: Eastern Cooperative Oncology Group performance status.

## FAQ

**What is a good IPI score?**
0–1.

**What is the NCCN-IPI?**
A refined version that weights age and LDH more finely.

**Does the IPI guide treatment?**
It informs prognosis and clinical trial eligibility rather than standard first-line treatment.
