Ejection fraction is the most widely used measure of heart pumping function. It defines the types of heart failure: HFrEF (EF ≤ 40%), HFmrEF (41–49%) and HFpEF (≥ 50% with symptoms).
How to use the Ejection Fraction Calculator
- Input
- End-diastolic volume
- End-systolic volume
- LV internal diameter, diastole
- LV internal diameter, systole
Results update as you type, and every field accepts the units shown in its menu.
How it works
Choose volumes (from Simpson’s biplane echo or MRI) or diameters (from M-mode echo). With diameters the calculator converts each diameter into a volume with the Teichholz formula first.
Inputs explained
| Field | What to enter |
|---|---|
| Input | Choose one: Volumes (EDV and ESV), Diameters (LVIDd and LVIDs) |
| End-diastolic volume | In mL |
| End-systolic volume | In mL |
| LV internal diameter, diastole | Units: cm, mm |
| LV internal diameter, systole | Units: cm, mm |
What each result means
- Ejection fraction = ([End-diastolic volume used] – [End-systolic volume used])÷[End-diastolic volume used]×100
- Stroke volume = [End-diastolic volume used] – [End-systolic volume used]
Example values
How the main result changes when End-diastolic volume changes and everything else stays at the example values above:
| End-diastolic volume (mL) | Ejection fraction (%) |
|---|---|
| 60 | 17 |
| 90 | 44 |
| 120 | 58 |
| 150 | 67 |
| 180 | 72 |
| 240 | 79 |

Formula
EF = (EDV − ESV) ÷ EDV × 100
Teichholz volume V = 7 ÷ (2.4 + D) × D³ (D in cm)
Normal LVEF ≥ 50% (ASE: 52–72% men, 54–74% women)
Worked example
Inputs — Input: Volumes (EDV and ESV); End-diastolic volume: 120 mL; End-systolic volume: 50 mL.
Results — End-diastolic volume used = 120 mL; End-systolic volume used = 50 mL; Ejection fraction = 58 %; Stroke volume = 70 mL.
Key terms
- LVEF
- Left ventricular ejection fraction.
- HFrEF
- Heart failure with reduced ejection fraction.
- HFpEF
- Heart failure with preserved ejection fraction.
Frequently asked questions
What is a normal ejection fraction?
50–70%; ASE gives 52–72% for men and 54–74% for women.
Can you have heart failure with a normal EF?
Yes – heart failure with preserved ejection fraction (HFpEF) is common, especially in older women.
Can EF improve?
Yes, often with treatment of the cause and modern heart failure medicines.
