The ICH score was published by Hemphill in 2001 and is the most widely used prognostic score for brain haemorrhage. It should support, not replace, clinical judgement, and early do-not-resuscitate decisions based on it alone are discouraged.
How to use the ICH Score Calculator
- Glasgow Coma Scale
- ICH volume ≥ 30 mL
- Intraventricular haemorrhage
- Infratentorial origin
- Age 80 or over
Results update as you type, and every field accepts the units shown in its menu.
How it works
Each answer carries the points shown in the published score. The calculator adds them up and maps the total to the published interpretation.
Scoring table
| Item | Answers and points |
|---|---|
| Glasgow Coma Scale | 13–15 = 0; 5–12 = 1; 3–4 = 2 |
| ICH volume ≥ 30 mL | No = 0; Yes = 1 |
| Intraventricular haemorrhage | No = 0; Yes = 1 |
| Infratentorial origin | No = 0; Yes = 1 |
| Age 80 or over | No = 0; Yes = 1 |
Inputs explained
| Field | What to enter |
|---|---|
| Glasgow Coma Scale | Choose one: 13–15, 5–12, 3–4 |
| ICH volume ≥ 30 mL | Choose one: No, Yes |
| Intraventricular haemorrhage | Choose one: No, Yes |
| Infratentorial origin | Choose one: No, Yes |
| Age 80 or over | Choose one: No, Yes |
What each result means
- ICH score = [Glasgow Coma Scale] + [ICH volume ≥ 30 mL] + [Intraventricular haemorrhage] + [Infratentorial origin] + [Age 80 or over]
- 30-day mortality = interp([ICH score], 0, 1, 0, 13, 26, 72, 97, 100, 100)

Formula
Score = sum of the points for each item
Worked example
Inputs — Glasgow Coma Scale: 13–15; ICH volume ≥ 30 mL: No; Intraventricular haemorrhage: No; Infratentorial origin: No; Age 80 or over: No.
Results — ICH score = 0 / 6; 30-day mortality = 0 %.
Key terms
- ICH
- Intracerebral haemorrhage.
Frequently asked questions
How is ICH volume measured?
With the ABC/2 method on CT – see the ICH volume calculator.
What is a high ICH score?
3 or more carries a high mortality in the original study.
Has treatment improved outcomes?
Yes – modern care has lowered mortality at each score level.
